Virtual Direct Primary Care (DPC) Membership Approval Visit
Required Before Enrollment
Enrollment in Direct Primary Care at Optimize by JaeNix is by provider approval only.
Before joining the DPC membership, all prospective patients must complete a required virtual approval visit. This visit allows us to review your medical history, current medications, expectations, and overall eligibility to ensure this model of care is appropriate for you.
DPC is designed for patients seeking general adult primary care and is not suitable for all medical needs.
What This Visit Includes
During your virtual DPC approval visit, we will:
• Review your full medical history
• Review current diagnoses and medications
• Discuss the scope and limitations of DPC care
• Review exclusions and non-covered services
• Confirm state eligibility for virtual care
• Determine whether DPC is an appropriate fit
• Answer questions before enrollment
This visit is conducted by secure phone or video and serves as both an intake and eligibility review.
Completion of this visit does not guarantee approval for membership.

Who Is NOT Eligible for DPC Membership
To avoid confusion, the following conditions and services are excluded from the DPC membership and may result in non-approval:

• ADHD stimulants
• Benzodiazepines
• Opioids
• Phentermine
• Controlled sleep medications
• Suboxone, buprenorphine, or methadone
Medical Complexity
• Insulin initiation or insulin pump management
• Unstable or complex chronic disease
• Hospital-level or specialty-level care
• Addiction medicine or detox services
Mental Health
• New psychiatric medication initiation
• Crisis-level mental health care
• Controlled psychiatric
medications
Other Exclusions
• Pediatric care
• Emergency care
• Medicare or Medicaid beneficiaries
DPC is not appropriate for patients seeking medication-only care, controlled substances, or specialty treatment access through primary care.
Important Membership Disclosures
• DPC is a cash-pay medical membership, not insurance
• DPC does not replace emergency care
• Visits are virtual and scheduled through the patient portal
• Membership includes up to two virtual visits per month
• Provider response times are within 24 business hours
• Labs, imaging, and medications are billed separately
• Membership requires a 90-day minimum commitment
• Enrollment is at provider discretion
