Payment Options

At Optimize by JaeNix, we offer flexible payment options for your convenience. Accepted forms of payment include:

  • Cash (in-clinic only)
  • Apple Pay
  • Zelle
  • Credit & Debit Cards
  • Cherry Payment Plans – Buy now, pay later financing available for select treatments


Processing & Financing Fees

To offset transaction and financing costs, the following fees apply:

  • Credit/Debit Card Payments: 3.5% processing fee
  • Payment Plans / Financing: 5% financing surcharge applied to the total amount financed

To avoid processing fees, payment may be made via cash, Zelle, or Apple Pay.

Booking Deposit & Payment Application

  • A $100 booking deposit is required at the time of scheduling to reserve provider time
  • valid card on file is required for all appointments, programs, and memberships
  • Full payment is required prior to services rendered
  • Booking fees cannot be transferred to future appointments


Booking Deposit Refund Policy

  • Cancellations made 24 hours or more in advance:
    The $100 booking deposit is 
    fully refunded
  • Cancellations made less than 24 hours in advance:
    The $100 booking deposit is 
    forfeited


How the $100 Booking Deposit Is Applied

Aesthetic Procedures & Injectable Treatments

  • All aesthetic services require a current Good Faith Exam (GFE)
  • If a GFE has been completed within the last 12 months, the full $100 is applied to your appointment
  • If no current GFE is on file:
  • $30 is applied toward the GFE
  • $70 is applied toward the treatment


Membership Consultations

  • All memberships require a consultation
  • Consultation fee: $100
  • If you enroll in a membership within 7 days of appointment:
  • $50 is applied to your first month
  • $50 is retained as the consultation/provider time fee


Discovery Intake Policy

The Discovery Intake is a required medical evaluation prior to enrolling in hormone, weight-loss, or peptide programs.

  • Fee: $395 (due at booking)


Credit Application

  • If you enroll in one of the following Optimization Packages within 7 days, the $395 will be applied as a credit:
  • Hormone Optimization Initiation Program (12 Weeks)
  • Metabolic Reset Program (Hormones + GLP-1)


If enrollment does not occur within 7 days, the $395 cannot be applied
This credit does 
not apply to standard monthly hormone memberships


Programs & Packages

  • All packages require:
  • Signed agreement
  • Card on file
  • Auto-pay authorization
  • Package fees are non-refundable once care has begun
  • Missed appointments, delayed follow-ups, or non-compliance do not pause or extend program timelines


Monthly Memberships

  • Require:
  • Auto-pay
  • Card on file
  • 3-month minimum commitment
  • After the initial term, memberships may be canceled with 7 days’ written notice
  • Membership pricing does not include pharmacy costs unless otherwise stated


Card on File Requirement

  • A valid card on file is required at all times
  • If your card expires, is lost, or is replaced, it is your responsibility to update payment information
  • Failure to maintain a valid payment method may result in delayed or paused care


Late & Failed Payments

Late Fees

  • Payments not received within 3 calendar days of the due date will incur a $25 late fee


Non-Payment Enforcement

  • Accounts 7 days past due may result in:
  • Unenrollment from membership programs
  • Paused care
  • No prescription refills until the account is brought current
  • A new valid card must be on file before services resume


By enrolling, you authorize Optimize by JaeNix to charge applicable fees to the card on file.


Medications & Pharmacy Services

  • All prescriptions require prepayment prior to pharmacy submission
  • Medications ship directly to the patient
  • Pharmacy fees are non-refundable once submitted


Cancellation & No-Show Policy

Appointment times are reserved exclusively for you.


Cancellations

  • At least 24 hours’ notice is required to cancel or reschedule
  • Cancellations within 24 hours result in forfeiture of the booking deposit
  • Appointments canceled within 24 hours or missed without notice will be charged in full


No-Show Appointments

Failure to arrive without notice will result in:

  • Forfeiture of the $100 booking deposit
  • Appointments missed without notice will be charged in full


Future appointments after a no-show may require full prepayment.

By booking an appointment or enrolling in a program, you acknowledge that you have read, understood, and agreed to this Payment & Cancellation Policy.
